Can Medicare be sustained in a climate of high and rising health care costs? Are there proposals that might improve quality of care while containing costs? A June 3 briefing, Medicare for the 21st Century, will address the sustainability of Medicare under its current design. A panel of Medicare experts reviewed proposals and provided perspectives on modernizing Medicare to improve the efficiency of the program, improve the quality of care and reduce overall health care costs. 
10Briefing  Program Integrity: How Much Money Can Combating Health Care Fraud and Abuse Save? , 3/5/2012
Headlines regularly call attention to pockets of fraudulent activity in the health care area--scams that amount to millions and potentially billions of dollars. The stories typically focus on catching the crooks but not so much on efforts to prevent fraud, waste and abuse in health care programs. Both types of efforts are important. With continued concerns about rising health care costs and the current focus on deficit reduction, how much money can be saved and put to better use by reducing waste, abuse and outright fraud? To address these questions and more, the Alliance for Health Reform and the Centene Corporation sponsored a March 5 briefing. The focus was not so much on the criminal aspect of the problem, but rather on how to coordinate the efforts of Medicaid, Medicare, the private sector and others to prevent and combat the waste, fraud and abuse. 
10Briefing  Integrating Care for Dual Eligibles: What Do Consumers Want? , 12/12/2011
The 9 million beneficiaries dually eligible for Medicare and Medicaid are generally poorer and sicker than other Medicare beneficiaries, tend to use more health care services, and thus account for a disproportionate share of spending in both programs. Many deficit reduction plans under recent discussion have recognized the need to improve care for this population and provide care in a more cost-effective way.How do Medicaid and Medicare coordinate payment and care for people covered by both programs? Are HHS initiatives encouraging innovations to integrate care for dual eligible beneficiaries? What kinds of programs are currently available? What do consumers think about fee-for-service plans, special needs plans or other programs in which they are currently enrolled? What lessons for program design can policy makers learn from the attitudes of consumers? To answer these and related questions, the Alliance for Health Reform and the AARP Public Policy Institute sponsored a December 12 briefing.  
10Briefing  Inside Deficit Reduction: What It Means for Medicare, 10/11/2011
Proposals to generate Medicare savings abound, from the various commissions recommending change, members of Congress and others. Which proposals will, or should, receive serious consideration by the super committee in its quest to find $1.2 trillion or more in savings by its November 23 deadline? What impact would these changes have on beneficiaries, providers and insurers? Would stakeholders prefer the automatic, but capped, reductions in the sequester to any recommendations on Medicare reductions the super committee might make? To address these questions and more, the Alliance for Health Reform and four cosponsors presented an October 11 briefing. Cosponsors were The Commonwealth Fund, the Kaiser Family Foundation, the Robert Wood Johnson Foundation and The SCAN Foundation.  
10Briefing  The Innovation Center: How Much Can It Improve Quality and Reduce Costs – and How Quickly?, 7/18/2011
The new Center for Medicare and Medicaid Innovation seeks to test innovative health care payment and service delivery models that can potentially enhance quality of care for beneficiaries while reducing costs. How is CMMI planning to administer its $10 billion in funding? What early projects is the center undertaking? These questions and others were addressed at a July 18 briefing cosponsored by The Commonwealth Fund.  
10Briefing  Caring for People Covered by Both Medicare and Medicaid: A Primer on Dually Eligible Beneficiaries, 6/3/2011
This was an introductory session designed to inform the staff of new members of Congress both in Washington and in district or state offices about the people who receive benefits from both the Medicaid and Medicare programs (often called “dual eligibles”). The briefing was designed to be helpful to staff members unfamiliar with this important issue. A distinguished panel addressed such basic questions as: Who is dually eligible for Medicare and Medicaid? What are the characteristics and needs of this population? How do Medicaid and Medicare coordinate payment and care for this population? What federal and state barriers complicate these efforts? What is being done to address these challenges? This briefing, cosponsored by the Kaiser Family Foundation's Commission on Medicaid and the Uninsured (KCMU), answered these and other questions.

Understanding how Medicare works is essential to understanding federal health policy. The Patient Protection and Affordable Care Act of 2010 affects Medicare in a number of ways, including benefit improvements, changes in payments to Medicare plans and providers, revenues and delivery system reforms. What is Medicare and what is its role in the health care system? How does the health reform law affect Medicare, and what is Medicare's role in accomplishing health reform's goals? To address these questions and more, the Alliance for Health Reform and the Kaiser Family Foundation sponsored a March 11 briefing.

10Briefing  Medicare Advantage: Lessons for the Future, 5/4/2009
The Alliance for Health Reform and the Kaiser Family Foundation sponsored a luncheon briefing on the basics of the Medicare Advantage program. Speakers included: Jonathan Blum, Centers for Medicare and Medicaid Services; Marsha Gold, Mathematica Policy Research; and Mark Miller, MedPAC. Ed Howard of the Alliance and Tricia Neuman of the Foundation were co-moderators.

Medicare covers nearly 45 million beneficiaries, including 38 million seniors and 7 million younger adults with permanent disabilities. The program is expected to cost the federal government approximately $477 billion in 2009, accounting for 13 percent of federal spending and 19 percent of total national health expenditures. Whom does Medicare serve and what services does it cover? What are Medicare Parts A, B, C and D? How is it structured and financed? What drives Medicare’s costs? How does Medicare reimburse providers and hospitals? What future challenges face the program? To address these and related questions, the Alliance for Health Reform and the Kaiser Family Foundation sponsored a March 16 briefing.

Medicare covers nearly 44 million beneficiaries who are elderly, including 37 million seniors and 7 million younger adults with permanent disabilities, and end stage renal disease. The program cost the federal government approximately $375 billion in 2006, accounting for 13 percent of federal spending.  
10Briefing  BRIDGING THE DIVIDE: Medicare's Role in Reducing Racial and Ethnic Disparities, 1/29/2007
A growing body of evidence shows disparities in quality of care among Medicare beneficiaries of different racial and ethnic backgrounds. These disparities are particularly noteworthy in Medicare, which provides nearly universal access to care to the elderly without regard to race or ethnicity.  
10Briefing  Medicare Advantage: Early Views and Trendspotting, 5/19/2006
The Medicare Prescription Drug, Improvement, and Modernization Act (MMA) of 2003 made a number of changes to the Medicare managed care program – Medicare Advantage. Among the aims: Keep managed care plans from leaving the program by offering higher payments, and give beneficiaries more reasons to consider joining. Is the Medicare Advantage program now growing again after a period of declining enrollment? Are additional changes to payments for Medicare managed care plans warranted?

10ToolkitThe Sustainable Growth Rate: Seeking a ‘Doc Fix’, 4/5/2013
The public is keeping a close eye on federal budget deficit reduction efforts this year, including potential automatic spending cuts initially mandated by the Budget Control Act of 2011. Yet one component of the debate has been largely ignored - the Sustainable Growth Rate (SGR). Indeed, because of the SGR, physicians in January 2013 faced a 26.5 percent cut in Medicare reimbursement rates. Last-minute congressional intervention delayed the cut until January 2014 as part of the American Taxpayer Relief Act of 2012. Without intervention, physicians will receive a 25 percent reimbursement cut in January 2014. At the same time, according to the most recent Congressional Budget Office (CBO) estimates, if Congress and the president agree to permanently eliminate the SGR, the deficit will grow by another $138 billion over 10 years. The cost of repealing the SGR has fallen significantly since last year, spiking a new interest in permanently fixing the problem. (Adobe Acrobat PDF)

10ToolkitToolkit: Medicare Private Fee-for-Service Plans, 7/13/2007
Over the past three years, enrollment in Medicare private fee-for-service (PFFS) plans has increased significantly. These plans offer a potentially greater choice of providers than beneficiaries will find in Medicare HMOs or PPOs. They often provide extra benefits not found in traditional Medicare. Beneficiaries attracted to the plans hope to lower their out-of-pocket costs compared to what they would pay in traditional Medicare. However, the plans have drawn the interest of federal budget cutters since they cost more per beneficiary than traditional Medicare. Moreover, beneficiaries have been reporting confusion about the plans and sometimes, enrollment fraud. Some private fee-for-service beneficiaries have been denied services by physicians who previously accepted their traditional Medicare coverage. This toolkit, supported by the Robert Wood Johnson Foundation, contains resources that describe the basics of PFFS plans, advantages and incentives included in the plans, and the challenges that PFFS enrollees have encountered along the way.  (Adobe Acrobat PDF)

There is widespread agreement that the current health care delivery system is fragmented. Your primary care physician may be the last to know what your cardiologist is doing, or your radiologist or pharmacist, for that matter. However, change is in the works. Both the private and public sectors are testing various care delivery transformation models to improve quality, reduce morbidity and mortality, and contain the costs of treatment. The alphabet soup includes Accountable Care Organizations (ACOs), Patient Centered Medical Homes (PCMHs) and other models. What do these acronyms signify? Which care coordination models is CMS supporting? Has the Innovation Center found any promising practices that may be scalable? Have these models shown a reduction in spending? Improvements in quality of care? How is improvement being measured? To answer these important questions and related ones, the Alliance for Health Reform and WellPoint, Inc. sponsored a September 10 briefing.

After much heated debate on the U.S. debt limit, the Budget Control Act of 2011, containing more than $900 billion in spending reductions over 10 years, was passed on August 2. What exactly is called for in the law? What are the implications for health care programs, including Medicare, Medicaid, CHIP and the Patient Protection and Affordable Care Act? To address these questions and more, the Alliance for Health Reform and four cosponsors presented a September 12 luncheon briefing. Cosponsors were The Commonwealth Fund, the Kaiser Family Foundation, the Robert Wood Johnson Foundation and The SCAN Foundation.  
9Briefing  Pathways to Payment Innovation in a Post-Health Reform Era, 5/10/2010
This briefing explored the major provider payment initiatives in the new health reform law. Topics discussed included demonstration projects included in the law, the new Center for Medicare and Medicaid Innovation, and the real-world practicalities involved in changing the way providers are paid. Cosponsored by The Commonwealth Fund, a philanthropy based in New York City and Washington, DC.  
9Briefing  Health Legislation 2007-2008: What's Possible?, 12/13/2006
Suddenly, Capitol Hill is talking about health issues again. Incoming House Speaker Nancy Pelosi promises that Democrats will roll back the prohibition against the federal government negotiating drug prices on behalf of Medicare beneficiaries. (A Kaiser Family Foundation poll released today found broad bipartisan support for this idea.) Incoming Senate Majority Leader Harry Reid says one of his three top priorities will be more funding for stem cell research. Others want to revisit the importation of prescription drugs from Canada. Additionally, the 110th Congress will consider whether to continue the State Children’s Health Insurance Program in its present form.  
9Briefing  Helping the Medicare Savings Programs Get Savings to Seniors, 6/12/2006
The Medicare Savings Programs provide assistance with premiums — and in some instances, cost sharing requirements — to Medicare beneficiaries of limited income and resources who do not qualify for full Medicaid benefits. What lessons does Part D enrollment offer that could apply to the savings programs? What techniques are working well for the savings programs that could apply to Part D enrollment? How could newly released recommendations from The National Academy of Social Insurance (NASI) improve the Medicare savings programs if implemented? How feasible are these recommendations?

The portion of Medicare enrollees in Medicare Advantage plans is on the upswing, now estimated at 19 percent of beneficiaries. By 2016, more than one in four beneficiaries may be covered by a Medicare Advantage plan. This briefing covered the costs and benefits of the Medicare Advantage program, and implications of overpayments to Medicare Advantage plans. Among questions discussed: Are the per-beneficiary overpayments (compared to traditional Medicare) worth it? What would happen if these payments were cut? Its ability to align incentives through pay-for-performance will have implications for the health delivery system at large. How do these countries decide which drugs will be approved and included in their prescription drug programs for the elderly and disabled?

Medicare beneficiaries can start signing up for prescription drug plans on November 15. Already, health plans have announced that they will be spending up to $80 million each to promote their offerings. The federal government and nonprofit groups are well into a massive effort to educate the public. But beneficiaries and those they rely on still have many questions about the program. The Alliance for Health Reform sponsored a briefing on Sept. 27 to help reporters with their coverage of the new benefit's implementation and offer story ideas.

The costs of caring for the elderly have been growing steadily higher. Spending on Medicare benefits accounted for 17 percent of the nation’s total health care spending in 2004. With baby boomers retiring, and Medicare benefits changing, this percentage is expected to grow in the future. How concerned should we really be with rising health care costs, especially costs connected with the care of older people? Will new technologies create efficiencies that will dramatically reduce costs, or will technologies raise demand for care and thus drive cost increases?

Medicare covers nearly 42 million beneficiaries who are elderly, or have a disability or end stage renal disease. The Medicare Modernization Act of 2003 made many changes to the program including the addition of a prescription drug benefit (“Part D”), which will begin full implementation in 2006. Whom does Medicare serve and what services does it provide? How is the program financed? What are the basics of the new Medicare drug benefit? To help answer these questions and more, the nonpartisan Alliance for Health Reform and the Kaiser Family Foundation held a May 16 luncheon briefing for those new to the topic of Medicare and those who wanted to refresh their knowledge.

The federal government’s responsibility to provide access to health care for the nation’s 41 million Medicare beneficiaries implies another obligation: to spend taxpayer dollars wisely. This means assuring that the $250 billion+ spent for Medicare goes for services that are safe, timely and effective. How well is the Medicare program meeting these goals? Why is there an inverse relationship between how much is spent per Medicare beneficiaries across the states and the quality of care rankings for those states? Why are there unexplained variations in care at the end of life? What is the relationship between staffing levels and quality? What should be done to boost the quality of care for Medicare beneficiaries while assuring the most effective use of resouces?

Is there any clear relationship between what hospitals are paid for services and quality of care?

President Bush’s FY 2006 budget could have long-lasting effects on several health programs. If enacted as is, the budget would trim $60 billion in the growth in Medicaid spending over the next 10 years. At the same time, the budget offers $11 billion in new money to enroll children in Medicaid and the State Children’s Health Insurance Program. It would provide $74 billion in tax incentives to help the uninsured buy coverage. Some congressional budget leaders have signaled their intention to find budget savings in another entitlement program – Medicare. After the hearings are over and the votes taken, how many of the president’s health initiatives will remain? What budget proposals originating with Congress might be adopted? How will discretionary programs – like the Centers for Disease Control and Prevention, and the National Institutes of Health – be affected? What are the long-term implications for health care providers and consumers, and for the deficit?

House Speaker-to-be Nancy Pelosi wants a vote early on to roll back the prohibition against the federal government negotiating prescription drug prices for Medicare beneficiaries. And these are just two of the health issues likely to be debated in the new Congress. The White House too will have a definite voice in the debates to come.

Although Medicare reform is not currently a front-burner issue, proposals to reduce Medicare spending appear regularly on the policy agenda. Various Medicare savings proposals have recently emerged in the context of efforts to control the national deficit and debt, and could arise in the next few months when Congress considers how to modify Medicare’s physician payment policy to avoid a precipitous reduction in physician fees. The recently passed bipartisan budget deal delayed a reduction in Medicare payments to physicians until April, and any effort to permanently replace the existing system by which Medicare pays physicians will be costly.

A new Alliance for Health Reform video features Princeton’s Uwe Reinhardt questioning whether bundling payments for medical services might actually lead to higher – not lower – costs.

"The ACO's, the accountable care organizations, could create local monopolies that could dictate to you what that bundled price would be, and some of us fear that bundled prices might be even more than what the fee-for-service for that bundle would be today. … You really should align all the payers and say, 'Let us jointly negotiate with the ACOs what those bundles should be so that they cannot divide and rule and sort of make us on the buy side weak.'"

Sister Carol Keehan on Health Law Enrollment Challenges this Year

Jonathan Blum on CMS Efforts to Keep Medicare Spending Growth Down

Jonathan Blum, acting principal deputy administrator and director of the Center for Medicare at the Centers for Medicare and Medicaid Services (CMS), addresses the slower growth of Medicare spending over the last few years, and what his agency is doing to try and continue the trend. “There are promising signs that this strategy to change the payment system, to change the payment models, to focus on waste and abuse, is paying off," he said. "We are taking a whole new approach to addressing fraud in the program. Much more data resources, much more on the ground reaction. Two Former Medicare Administrators Discuss How to Save the Program


A new Alliance for Health Reform video features two former Medicare administrators -- Gail Wilensky and Bruce Vladeck -- on their ideas about how to save the program.

Ms. Wilensky, who ran Medicare in a Republican administration, argued for increasing the eligibility age of Medicare for future retirees, while still making the program available at age 65 for those who are disabled.

Douglas Holtz-Eakin: Health Care Spending Lull Only Temporary

A new Alliance for Health Reform video features Douglas Holtz-Eakin of the American Action Forum, and a former director of the Congressional Budget Office, arguing that recent slower spending growth in health care won’t continue.

"We also saw a slowdown in the mid-90s, and we all declared victory and it came right back. I think next year we’ll see a noticeable uptick. There will be lots of new people entering the insurance markets because of the exchanges and the subsidies that come along with them, and those subsidies are very generous. … These are an invitation for people to get coverage and to buy more health care. I think that’ll place a lot of pressure on spending."
