Can Medicare be sustained in a climate of high and rising health care costs? Are there proposals that might improve quality of care while containing costs? A June 3 briefing, Medicare for the 21st Century, will address the sustainability of Medicare under its current design. A panel of Medicare experts reviewed proposals and provided perspectives on modernizing Medicare to improve the efficiency of the program, improve the quality of care and reduce overall health care costs. 
10Briefing  Health Care Costs: What Can Be Done?, 6/12/2012
Though there is still disagreement about the extent to which various cost drivers contribute to the troubling trajectory of health care spending in the United States, there are success stories. This briefing took a look at some of the innovative strategies in both the public and private sectors that have bent the cost curve downwards and some that may hold promise for lowering the rate of growth of national health care spending. The briefing also featured Dr. Paul Ginsburg presenting a paper that was commissioned especially for this series. The paper examines a range of policy strategies that might promote changes in health financing and delivery that would encourage higher quality and more efficient care delivery. 
10Briefing  Health Care Costs: The Role of Technology and Chronic Conditions, 5/29/2012
Many factors have been cited as health care cost drivers, including demography, geography, economics and health status. But there is little agreement on how much each factor contributes to overall costs. This briefing offered an in-depth look at two of the most-often cited cost drivers -- technology and chronic conditions. Cosponsored by the Alliance and nine other groups.  
10Briefing  Essential Health Benefits: Balancing Affordability and Adequacy , 2/3/2012
Under the Patient Protection and Affordable Care Act (PPACA), insurance plans offered through state insurance exchanges – as well as non-grandfathered plans offered in the individual and small group markets – will be required to cover a set of health benefits and services called the “essential health benefits” package. Guidance issued last month by the Department of Health and Human Services will give each state some discretion to specify benefits within the 10 categories specified in the law. The implementation of the essential health benefits package raises many questions. What is the task before states in selecting and designing “benchmark” plans that are comprehensive but also affordable? What does it mean for health plans to have the ability to offer benefits that are “substantially equal” to the benchmark plan? How will states incorporate state benefit mandates? What recommendations did the Institute of Medicine make on ensuring that health plans are affordable over time? To address these and related questions, the Alliance for Health Reform and The Commonwealth Fund sponsored a February 3 briefing.  
10Briefing  Inside Deficit Reduction: What It Means for Medicare, 10/11/2011
Proposals to generate Medicare savings abound, from the various commissions recommending change, members of Congress and others. Which proposals will, or should, receive serious consideration by the super committee in its quest to find $1.2 trillion or more in savings by its November 23 deadline? What impact would these changes have on beneficiaries, providers and insurers? Would stakeholders prefer the automatic, but capped, reductions in the sequester to any recommendations on Medicare reductions the super committee might make? To address these questions and more, the Alliance for Health Reform and four cosponsors presented an October 11 briefing. Cosponsors were The Commonwealth Fund, the Kaiser Family Foundation, the Robert Wood Johnson Foundation and The SCAN Foundation.  
10Briefing  Managing Costs and Improving Care: Team-based Care of the Chronically Ill, 8/11/2011
Treating those with multiple chronic conditions, including the elderly and disabled populations, accounts for 30 percent of total U.S. health care spending as of 2010. Half of this amount is spent by Medicare and Medicaid on behalf of beneficiaries eligible for both programs. It may be possible to improve the quality of care for the chronically ill while altering the trajectory of spending for their care. Savings have been shown in some private and public sector approaches using teams that span multiple sites of care, reduce fragmentation and improve health outcomes. How can Medicare, Medicaid, private plans and providers partner to develop new approaches and achieve these public health goals? This briefing, cosponsored by The Commonwealth Fund, addressed these questions and more.  
10Briefing  The Innovation Center: How Much Can It Improve Quality and Reduce Costs – and How Quickly?, 7/18/2011
The new Center for Medicare and Medicaid Innovation seeks to test innovative health care payment and service delivery models that can potentially enhance quality of care for beneficiaries while reducing costs. How is CMMI planning to administer its $10 billion in funding? What early projects is the center undertaking? These questions and others were addressed at a July 18 briefing cosponsored by The Commonwealth Fund.  
10Briefing  SCORING SAVINGS: How Can Quality Improvement Reduce Health Care Costs?, 7/13/2009
As the health reform debate heats up in summertime Washington, one key consideration is how to provide better value for the health care dollars spent. This briefing, cosponsored by the United Health Foundation, addressed questions such as: How can the private sector contribute to a slowing of national health care expenditures? Are there examples of models that have been implemented in the marketplace and have demonstrated they can save money? 10Briefing  Measuring up: A Comprehensive Scorecard for America's Health System, 10/11/2006
A new assessment by The Commonwealth Fund finds that deficiencies in the nation’s health care system cost 100,000 to 150,000 lives and $50 to $100 billion annually. The scorecard compares national averages for the U.S. to national and international benchmarks for 37 indicators. The U.S. received a composite score of 66 out of 100. On one quality measure, mortality, the U.S. ranked 14th out of 19 countries. 
10Briefing  BALANCING THE PROMISE AND COST OF BIOTECHNOLOGY , 9/22/2006
Biotechnology accounts for only one percent of insurers’ costs, but those costs are growing at a double digit rate. As science produces increasingly sophisticated and expensive medical products and procedures based on the manipulation of living organisms, payers will increasingly struggle with managing their use.

This issue brief examines chronic conditions as the biggest driver of health care costs, and the potential help of community intervention. Obesity, for example, is closely associated with several chronic conditions, and diabetes and other chronic conditions can often be prevented or delayed by addressing health-related behaviors such as smoking, physical inactivity and poor diet. 9Briefing  Worker Wellness Programs: Do They Work?, 5/31/2013
Starting in 2014, employers will be allowed to charge their workers up to 30 percent more for health insurance premiums if they don't meet certain health goals, under the Patient Protection and Affordable Care Act (ACA). An Alliance for Health Reform briefing, Worker Wellness Programs: Do They Work? explained the provisions in the law, and examined employer efforts to improve worker wellness, along with evidence about savings. What effect can tying wellness incentives to premiums have on workers, and especially less healthy workers? What does the evidence say about wellness programs and costs? Under the proposed agency rules related to the ACA provisions, what would be permitted and not permitted in wellness program design and practice? What are key concerns about implementing such programs? A distinguished panel of experts addressed these and related questions. 
9Briefing  The Alphabet Soup of Care Delivery Transformation, 9/10/2012
There is widespread agreement that the current health care delivery system is fragmented. Your primary care physician may be the last to know what your cardiologist is doing, or your radiologist or pharmacist, for that matter. However, change is in the works. Both the private and public sectors are testing various care delivery transformation models to improve quality, reduce morbidity and mortality, and contain the costs of treatment. The alphabet soup includes Accountable Care Organizations (ACOs), Patient Centered Medical Homes (PCMHs) and other models. What do these acronyms signify? Which care coordination models is CMS supporting? Has the Innovation Center found any promising practices that may be scalable? Have these models shown a reduction in spending? Improvements in quality of care? How is improvement being measured? To answer these important questions and related ones, the Alliance for Health Reform and WellPoint, Inc. sponsored a September 10 briefing. 
9Briefing  Chronic Disease Prevention: Saving Lives, Saving Money, 7/13/2012
With a continued focus on the need to control the high and rising cost of care, Congress is looking for low cost, high yield policy solutions. Innovative public health initiatives are attracting businesses, improving workplace wellness, and providing a spark to local economies, but is there evidence that public health investments can help prevent chronic disease and reduce escalating health care costs? And how does CBO view these with regard to cutting the deficit and/or affecting long-term costs/savings to the health care system? What can we learn from our experience with tobacco cessation programs? Have initiatives on the local level been effective? How can public health efforts intersect with clinical care? What new initiatives are being designed on the state level? Are these being supported by federal policy and federal funds? To address these questions and more, the Alliance for Health Reform and the Robert Wood Johnson Foundation sponsored a July 13 briefing. 
9Briefing  Coping with Fragmented Payment in the Real World, 4/13/2012
Many analysts and policymakers agree that the fragmentation of the health care delivery system results in uncoordinated care, frustrated patients, higher costs, wasted administrative dollars and lost opportunities for rapid improvement in our health care system. How should payments be reformed in order to harmonize health care delivery and reduce this fragmentation? To answer this important question and related ones, the Alliance for Health Reform and The Commonwealth Fund sponsored an April 13 briefing.  
9Briefing  Inside Deficit Reduction: What Now?, 12/2/2011
The Budget Control Act of 2011 tasked six senators and six representatives, from both sides of the aisle, to find at least $1.2 trillion in deficit reduction over the next decade. After weeks of deliberation, members of the “Super Committee” did not reach an agreement by the November 23 deadline. As a result, automatic spending cuts to defense and social welfare programs are set to kick in beginning in January 2013, leaving many questions about the short-term and long-term consequences of the committee’s failed negotiations. How will Congress and the executive branch react, especially in the context of a Presidential election campaign? What impact will a sequester have on the health care sector? Is Congress likely to devise alternatives by 2013, given such vast differences among lawmakers? How will implementation of the Patient Protection and Affordable Care Act be affected? To address these questions and more, the Alliance for Health Reform and four cosponsors presented a December 2 briefing. 
9Briefing  Inside Deficit Reduction: What it Means for Medicaid, 11/10/2011
Medicaid currently provides health and long-term care to 68 million Americans, and the Patient Protection and Affordable Care Act would significantly expand Medicaid beginning in 2014. As a countercyclical program, Medicaid typically faces increased enrollment at the same time that states are looking to cut their budgets. Although the Budget Control Act of 2011 exempts safety-net programs like Medicaid from across-the-board cuts, Medicaid has remained a keenly debated topic in discussions of how to reduce the deficit. Which proposals affecting Medicaid might receive serious consideration by the “super committee” in its quest to find $1.2 trillion or more in savings by its November 23 deadline? What impact would these changes have on the populations served by this program, providers, and states? To address these questions and more, the Alliance for Health Reform and four cosponsors presented a November 10 briefing. 
9Briefing  Keeping Coverage Continuous: Smoothing the Path between Medicaid and the Exchange, 5/20/2011
When fully implemented, the Patient Protection and Affordable Care Act will increase the number of insured Americans primarily through Medicaid expansion and the creation of state-based insurance exchanges. A key design challenge for those implementing the reform law is how to manage churning, when people cycle in and out of public programs as their income varies. What approaches are states and the federal government taking to minimize the disruption from churning? Will people be able to keep their provider as they move across the Medicaid-exchange divide? How can private insurers and Medicaid overcome possible technical and cultural barriers to a cooperative working relationship? To answer these questions and more, the Alliance for Health Reform and The Commonwealth Fund sponsored a May 20 luncheon briefing. 
9Briefing  Medicaid: A Primer on the Federal-State Partnership, 3/4/2011
Medicaid will play a major role in extending coverage to millions of uninsured people in 2014 through provisions in the Patient Protection and Affordable Care Act. More than 16 million newly qualified individuals are expected to enroll as a result of the law. What is Medicaid and what is its role in the health care system? Who is eligible and what benefits are covered? How is the program administered? How much does Medicaid cost and how is it financed? What is Medicaid's role in health reform? To address these questions and more, the Alliance for Health Reform and the Kaiser Commission on Medicaid and the Uninsured sponsored a March 4 briefing.  
9Briefing  Shared Medical Decision Making: We're in This Together , 2/14/2011
The relationship between patients and doctors has been changing from the traditional model in which a doctor decides on a treatment course without significant patient input. One way to facilitate shared decision making is through the use of decision support tools, which offer the patient high quality medical information in an easily understandable format. How do these tools work? How might shared decision making benefit patients, purchasers and providers of care? What role can shared decision making play in the larger effort to improve the quality and lower the cost of health care? How does the Affordable Care Act affect shared decision making? To address these and related questions, the Alliance for Health Reform, the National Committee for Quality Assurance and the Foundation for Informed Medical Decision Making sponsored a February 14 briefing. 
9Briefing  Affordability and Health Reform: If We Mandate, Will They (and Can They) Pay?, 11/20/2009
The health reform proposals being considered in the House and Senate may impose responsibilities on both individuals and employers to have, and help pay for, coverage. Subsidies for some small businesses and for individuals with incomes up to 400 percent of the federal poverty level have been proposed. But will individuals and businesses be able to pay the amounts required of them above the subsidies? This Nov. 20 briefing cosponsored by the Alliance and The Commonwealth Fund explored this question and related ones.  
9Briefing  Consumer Choice in Health Care: How Could Reform Affect Our Choices? How Could We Make Better Choices?, 6/25/2009
Now that health reform is being discussed in earnest on Capitol Hill, some wonder if we will be able to maintain our existing choices under a reformed health care system. Others maintain that our choices will grow, not shrink, as a result of reform, especially for the uninsured and the underinsured. Yet another viewpoint: Most Americans have choices now, but we don’t have enough information to make well-informed choices. This briefing, which was open to media only, brought together nationally known health policy experts for a conversation on consumer choice in health care. Cosponsored by the Robert Wood Johnson Foundation.  
9Briefing  Making the Case for Prevention: Tales from the Field, 6/19/2009
To improve the health of communities and the general population, an array of health reformers, states and businesses alike are all looking to a range of prevention measures such as chronic disease management, alcohol and smoking cessation, and obesity programs. The hope is that these measures will also improve value and control costs. What types of prevention models are working for business and government? What impact on health have prevention programs had? Does the evidence prove that prevention can cut costs? What emphasis should be placed on community vs. clinical prevention? What improvements need to be made in public health infrastructure, workforce and training to improve prevention efforts? To discuss these recommendations and questions, the Alliance for Health Reform and the Robert Wood Johnson Foundation sponsored a June 19 briefing.

Provider payment policy is a hot topic again with the Medicare physician payment system due for a fix and value-based payment and other cost containment/quality schemes on the table as part of the health reform debate. This briefing sponsored by the Alliance and The Commonwealth Fund examined cost containment options in payment reform.  
9Briefing  Treatment of Severe Chronic Illness: What Explains Cost and Quality Variations? Should We Be Concerned?, 9/8/2006
Some Medicare beneficiaries receive significantly more hospital-based services during the last two years of life than do other beneficiaries. The number of physician visits for Medicare beneficiaries can also vary greatly. What accounts for this variation? Do patients receiving more services tend to get better care, or not? Should policymakers take steps to more closely examine the relationship between spending and the volume of services provided at different facilities? 
9Briefing  HEALTH CARE in the 2006 BUDGET: What’s Next? What to Watch for in the Coming Debates, 2/18/2005
President Bush’s FY 2006 budget could have long-lasting effects on several health programs. If enacted as is, the budget would trim $60 billion in the growth in Medicaid spending over the next 10 years. At the same time, the budget offers $11 billion in new money to enroll children in Medicaid and the State Children’s Health Insurance Program. It would provide $74 billion in tax incentives to help the uninsured buy coverage. Some congressional budget leaders have signaled their intention to find budget savings in another entitlement program – Medicare. After the hearings are over and the votes taken, how many of the president’s health initiatives will remain? What budget proposals originating with Congress might be adopted? 9ToolkitThe Sustainable Growth Rate: Seeking a ‘Doc Fix’, 4/5/2013
The public is keeping a close eye on federal budget deficit reduction efforts this year, including potential automatic spending cuts initially mandated by the Budget Control Act of 2011. Yet one component of the debate has been largely ignored - the Sustainable Growth Rate (SGR). Indeed, because of the SGR, physicians in January 2013 faced a 26.5 percent cut in Medicare reimbursement rates. Last-minute congressional intervention delayed the cut until January 2014 as part of the American Taxpayer Relief Act of 2012. Without intervention, physicians will receive a 25 percent reimbursement cut in January 2014. At the same time, according to the most recent Congressional Budget Office (CBO) estimates, if Congress and the president agree to permanently eliminate the SGR, the deficit will grow by another $138 billion over 10 years. Oral Health and the Affordable Care Act: Only Part Way to the Finish Line, identifies provisions in the Patient Protection and Affordable Care Act that expanded dental coverage, primarily for kids, and called for education. But some provisions in the law have gone unfunded. Many adults currently lack dental coverage, and minorities and poor people are at the highest risk for disease. Some states are considering legislative proposals to train less expensive mid-level practitioners – somewhere between dentists and hygienists – to be able to do more. Spending on prescription drugs in the U.S. rose at a faster clip in 2009 than spending for hospital and physician care, a trend that is expected to continue through 2020. Thus, federal and state budget cutters – as well as private health plans and hospitals – have a keen interest in restraining drug costs. The desire to get a handle on drug spending is a focus in other countries as well. What are some strategies used in other countries to control pharmaceutical spending? How are value-based models being used by insurers and others to influence pharmaceutical utilization and health outcomes? How do the public and private sectors in the United States seek to contain pharmaceutical spending? To address these and related questions, the Alliance for Health Reform and The Commonwealth Fund are sponsoring a November 7 luncheon briefing. 
8Briefing  Medicaid Innovations: Can Managed Care Cut Costs and Improve Value?, 10/28/2011
According to a recent survey, a number of states have expressed interest in expanding managed care approaches within their Medicaid programs. But while managed care may present an opportunity for better delivery of care, it presents challenges within certain populations and geographic areas. What are the different models for managed care within the Medicaid program? What impact would these changes have on beneficiaries, providers and health plans? What have we learned from past efforts to expand managed care? How are states planning to use managed care to prepare for 2014, when 16 million newly qualified beneficiaries begin enrolling in Medicaid as authorized by the Patient Protection and Affordable Care Act? To address these questions and more, the Alliance for Health Reform and the Centene Corporation sponsored an October 28 briefing.

After much heated debate on the U.S. debt limit, the Budget Control Act of 2011, containing more than $900 billion in spending reductions over 10 years, was passed on August 2. What exactly is called for in the law? What are the implications for health care programs, including Medicare, Medicaid, CHIP and the Patient Protection and Affordable Care Act? To address these questions and more, the Alliance for Health Reform and four cosponsors presented a September 12 luncheon briefing. Cosponsors were The Commonwealth Fund, the Kaiser Family Foundation, the Robert Wood Johnson Foundation and The SCAN Foundation.  
8Briefing  Accountable Care Organizations: A New Paradigm for Health Care Delivery?, 5/13/2011
In addition to expanding coverage to 32 million previously uninsured Americans, the health reform law of 2010 aims to improve the health of the population and the quality of health care delivery, while reducing costs. One tool to pursue these goals is the creation of a national voluntary program for accountable care organizations (ACOs). The ACO provision and the proposed rule raise many questions. How can health care providers organize to provide more patient-centered, coordinated, accountable care? How will the new program affect the relationships among primary care providers, specialists, and hospitals? Will coordination among providers mean concentration of market power and higher prices? What lessons can be learned from existing ACOs about achieving better care, better health, and lower costs? To address these and related questions, the Alliance for Health Reform and The Commonwealth Fund sponsored a May 13 luncheon briefing.

Suddenly, Capitol Hill is talking about health issues again. Incoming House Speaker Nancy Pelosi promises that Democrats will roll back the prohibition against the federal government negotiating drug prices on behalf of Medicare beneficiaries. (A Kaiser Family Foundation poll released today found broad bipartisan support for this idea.) Incoming Senate Majority Leader Harry Reid says one of his three top priorities will be more funding for stem cell research. Others want to revisit the importation of prescription drugs from Canada. Additionally, the 110th Congress will consider whether to continue the State Children’s Health Insurance Program in its present form.

Over the past three years, enrollment in Medicare private fee-for-service (PFFS) plans has increased significantly. These plans offer a potentially greater choice of providers than beneficiaries will find in Medicare HMOs or PPOs. They often provide extra benefits not found in traditional Medicare. Beneficiaries attracted to the plans hope to lower their out-of-pocket costs compared to what they would pay in traditional Medicare. However, the plans have drawn the interest of federal budget cutters since they cost more per beneficiary than traditional Medicare. Moreover, beneficiaries have been reporting confusion about the plans and sometimes, enrollment fraud. Some private fee-for-service beneficiaries have been denied services by physicians who previously accepted their traditional Medicare coverage. This toolkit, supported by the Robert Wood Johnson Foundation, contains resources that describe the basics of PFFS plans, advantages and incentives included in the plans, and the challenges that PFFS enrollees have encountered along the way.  (Adobe Acrobat PDF)

As presidential candidates of both major parties woo the American public, particularly in early caucus and primary states, they are gravitating toward a common public concern: the U.S. health care system. Potential voters are well aware of rising premiums and higher out of pocket costs to families, the impact of high health care costs on America’s economic competitiveness and on federal and state budgets, and the ever-growing number of uninsured – now nearly 45 million. A Kaiser Family Foundation poll released in March shows that health care ranks as the second most important issue that the public would like discussed by presidential candidates for the 2008 election.

Health care policy leaders are counting on public and private initiatives, such as paying for performance, to improve value in the health care equation in which cost and quality at times seem to be at odds. Health care costs are high and quality outcomes are low in several metrics, compared to other developed nations. And, studies have shown that costs vary among states and within states, and that regions with higher costs do not necessarily have higher quality.

Employer-sponsored coverage rates have been steadily falling, from 66 percent of non-elderly Americans in 2000 to slightly below 60 percent in 2006. Experts cite rising premium costs and workforce changes as factors driving the erosion in such coverage. What are the pros and cons of employer-sponsored coverage? Who is losing coverage and why? Is it possible to maintain or improve health coverage for employees and also prevent jobs from moving overseas? A new Alliance for Health Reform video features Princeton’s Uwe Reinhardt questioning whether bundling payments for medical services might actually lead to higher – not lower – costs.

"The ACO's, the accountable care organizations, could create local monopolies that could dictate to you what that bundled price would be, and some of us fear that bundled prices might be even more than what the fee-for-service for that bundle would be today. … You really should align all the payers and say, 'Let us jointly negotiate with the ACOs what those bundles should be so that they cannot divide and rule and sort of make us on the buy side weak.'"

Sister Carol Keehan on Health Law Enrollment Challenges this Year

Jonathan Blum on CMS Efforts to Keep Medicare Spending Growth Down

Jonathan Blum, acting principal deputy administrator and director of the Center for Medicare at the Centers for Medicare and Medicaid Services (CMS), addresses the slower growth of Medicare spending over the last few years, and what his agency is doing to try and continue the trend. “There are promising signs that this strategy to change the payment system, to change the payment models, to focus on waste and abuse, is paying off," he said. "We are taking a whole new approach to addressing fraud in the program. Much more data resources, much more on the ground reaction. We have seen dramatic spending declines in areas of spending, such as home health and durable medical supplies that historically fueled lots of the fraud.”  FULL TRANSCRIPT

Two Former Medicare Administrators Discuss How to Save the Program


A new Alliance for Health Reform video features two former Medicare administrators -- Gail Wilensky and Bruce Vladeck -- on their ideas about how to save the program.

Ms. Wilensky, who ran Medicare in a Republican administration, argued for increasing the eligibility age of Medicare for future retirees, while still making the program available at age 65 for those who are disabled.

Douglas Holtz-Eakin: Health Care Spending Lull Only Temporary

A new Alliance for Health Reform video features Douglas Holtz-Eakin of the American Action Forum, and a former director of the Congressional Budget Office, arguing that recent slower spending growth in health care won’t continue.

"We also saw a slowdown in the mid-90s, and we all declared victory and it came right back. I think next year we’ll see a noticeable uptick. There will be lots of new people entering the insurance markets because of the exchanges and the subsidies that come along with them, and those subsidies are very generous. … These are an invitation for people to get coverage and to buy more health care. I think that’ll place a lot of pressure on spending."

Toolkit: Replacing the SGR


A new Alliance for Health Reform Toolkit summarizes the recent policy debate surrounding the decade-long problem In the last six months, the House Energy and Commerce Committee, House Ways and Means, and Senate Finance committees all put forth similar bills to repeal and reform the flawed Medicare physician payment formula. Further, with the CBO estimating the cost of repeal at $116.5, the lowest since 2006, many health policy experts are speculating that 2014 is the year that policymakers enact a permanent change to the SGR.
