This briefing, cosponsored by the Robert Wood Johnson Foundation, looked into how the states and the federal government are working together to implement the law -- both near-term provisions, such as high-risk pools, as well as other provisions farther down the road, such as setting up exchanges and instituting changes to Medicaid eligibility.

This briefing explored the major provider payment initiatives in the new health reform law. Topics discussed included demonstration projects included in the law, the new Center for Medicare and Medicaid Innovation, and the real-world practicalities involved in changing the way providers are paid. Cosponsored by The Commonwealth Fund, a philanthropy based in New York City and Washington, DC.

Many provisions in the new reform law concern private health insurance. As part of a series exploring what’s in the Patient Protection and Affordable Care Act, this briefing dealt exclusively with how the reform law affects access to private coverage, including the new federal high-risk pools, tax credits for small businesses, health insurance exchanges, the individual mandate and employer obligations. Cosponsored by the Henry J. Kaiser Family Foundation.

This briefing, cosponsored by the Kaiser Family Foundation, is part of our ongoing series to explore the provisions of the Patient Protection and Affordable Care Act (PPACA) and the Health Care and Education Reconciliation Act of 2010 (HCERA). A panel of well-qualified experts explained how the two health reform laws affect Medicaid, and to answered a variety of questions about topics such as Medicaid eligibility, financing, and other implementation issues. 
10Briefing  What's in There? An Ask-the-Experts Overview of the Health Reform Law, 4/16/2010
The Patient Protection and Affordable Care Act and the Reconciliation Act of 2010 are now law. Whether you favored or opposed their enactment, a full understanding of the new laws is essential to further discussion of reform issues. This briefing, cosponsored by The Commonwealth Fund, gave attendees a chance to ask questions of a well-qualified panel of experts.  
10Briefing  Health Insurance Exchanges: House or Senate Style?, 1/8/2010
Health insurance exchanges can potentially serve a variety of policy ends, from promoting transparency and competition among health plans, to pooling or reallocating risk and administering subsidies for those unable to afford health insurance premiums. This briefing, cosponsored by the Alliance and The Commonwealth Fund, looked at how the House and Senate reform bills deal with exchanges. One major difference is that the House proposal would set up a national exchange, with states having the right to opt out; the Senate version envisions state exchanges. What are the strengths and weaknesses of each approach? How do the respective bills address the issue of risk adjustment? Who is allowed to participate in the exchanges?  
10Briefing  Getting the Most Bang for Our Post-Health Reform Buck: Enrolling and Retaining Everyone Who’s Eligible, 12/1/2009
One of the main goals of health reform is to increase access to health care. Frequently overlooked in this effort is finding effective and efficient mechanisms for determining eligibility and enrolling those who are eligible for existing public programs, primarily Medicaid and the the Children's Health Insurance Program (CHIP). How can recruitment and enrollment be streamlined? How can enrollment in public programs affect states’ budgets? To answer these questions and others, the Alliance for Health Reform and PhRMA sponsored a December 1 briefing.  
10Briefing  Affordability and Health Reform: If We Mandate, Will They (and Can They) Pay?, 11/20/2009
The health reform proposals being considered in the House and Senate may impose responsibilities on both individuals and employers to have, and help pay for, coverage. Subsidies for some small businesses and for individuals with incomes up to 400 percent of the federal poverty level have been proposed. But will individuals and businesses be able to pay the amounts required of them above the subsidies? This Nov. 20 briefing cosponsored by the Alliance and The Commonwealth Fund explored this question and related ones.  
10Briefing  Public Opinion on Health Reform: What Do the Polls Mean?, 10/23/2009
Public support for health reform waxes and wanes depending, not only on what’s being proposed in the reform proposals, but also on who asks the question and how it is asked. Some recent polls indicate a majority of Americans support health care reform now; is that still the case? Do people want to pay for covering the uninsured – and if so, how much? To address these and related questions, the Alliance for Health Reform and the Kaiser Family Foundation sponsored an October 23 briefing. Downloadable resources available, including lists of experts and websites.  
10Briefing  The Uninsured: What Do the New Numbers Mean for Health Reform?, 9/18/2009
As Congress hashes out proposals to expand coverage to tens of millions of uninsured Americans, the latest count of the number of uninsured is a significant factor. Though some believe economic recovery is underway, Americans are still losing jobs by the hundreds of thousands each month. With the loss of jobs, so goes health insurance. How does this reality affect health reform and the notion of building on the current system? Has the complexion of who is uninsured changed? What has been the role of public programs in the recession? To address these and related questions, the Alliance for Health Reform and the Kaiser Family Foundation sponsored a September 18 briefing. 
10Briefing  Ask the Experts About Reform: Where Are We Now and Where Are We Headed?, 9/14/2009
Now that Congress has returned, we are clearly at a crucial stage in our consideration of how, and to what extent, we should reshape our health care system. What is the status of major reform bills? How do they differ from each other, and from the plan that may emerge from the Senate Finance Committee? What are some of the outstanding issues that need to be decided to get a bill to the president’s desk? This briefing, cosponsored by Eli Lilly, gave attendees a chance to pose whatever questions they liked about current health reform efforts on Capitol Hill.  
10Briefing  The Next 100 Days: Some Final Hurdles to Health Reform, 9/3/2009
Significant disagreements remain that must be worked out if health reform is to become law. Will there be a public plan option or, as an alternative, health insurance co-ops? How will reform be financed? Can the “cost curve” be bent, either during or after the 10-year window that’s the focus of Congressional Budget Office “scoring”? How might Medicare change – or facilitate change – in a reformed system? How will reform affect employers, especially small businesses? This briefing, cosponsored by the Robert Wood Johnson Foundation, looked at some of the important issues that must be settled if President Obama is to sign a meaningful health reform bill this year.

As the key congressional committees draft health reform legislation, they are keenly mindful of the costs of various provisions. Congress will look to an array of options, including both savings from the health care system and new revenues, to finance coverage expansions and reforms to both the delivery system and insurance markets. What options hold the most promise? How do the Congressional Budget Office (CBO) and the Office of Management and Budget (OMB) go about “scoring” various financing options? Are there credible options for financing reform? Is there room for bipartisan cooperation in paying for reform? For reporters who normally cover topics other than health reform, today’s reform debates can mean playing catch-up. They may be facing a new vocabulary – public plan option, health insurance cooperative, employer mandate, individual mandate. Maybe they're trying to figure out what all this means for readers, viewers or listeners. To help speed reporters along the learning curve, the Alliance for Health Reform sponsored a July 1 briefing. The webcast and resource materials compiled will be valuable for anyone wanting an overview of the health reform process in Washington to date, and where it's headed.

Now that health reform is being discussed in earnest on Capitol Hill, some wonder if we will be able to maintain our existing choices under a reformed health care system. Others maintain that our choices will grow, not shrink, as a result of reform, especially for the uninsured and the underinsured. Yet another viewpoint: Most Americans have choices now, but we don’t have enough information to make well-informed choices. This briefing, which was open to media only, brought together nationally known health policy experts for a conversation on consumer choice in health care. 10Briefing  Health Insurance Exchanges: See How They Run, 5/11/2009
Various proposals to expand coverage to uninsured Americans and reform the health insurance market include the establishment of a health insurance exchange. The most widely discussed example of such an exchange is in Massachusetts, and it arrived recently with Massachusetts’ 2007 health reform efforts. Many are looking to the Connector, as the Massachusetts program is known, to inform their discussions of a national insurance exchange. But is that the only model? What is actually meant by a health insurance exchange and how might it work? What’s in it for the consumer? How would vulnerable populations be protected? Who stands to benefit? To answer these and related questions, the Alliance for Health Reform and The Commonwealth Fund sponsored this May 11 briefing. 
10Briefing  Public Plan Option: Fair Competition or a Recipe for Crowd-Out? , 4/27/2009
One of the various proposals to expand coverage to the more than 45 million uninsured Americans is referred to as the “public plan option.” Though proposed only as part of a broader effort to expand coverage, the option is viewed in varying lights. Some see it as a tool for providing consumers affordable coverage by stimulating competition on the basis of quality and efficiency. Others see it as unfair competition for private insurers and, in the extreme, as a path to a single-payer system. What is actually meant by a public plan and how would it work? Is there evidence of fair competition from public plans? Might private plans outperform a public option? What are some examples of private systems today that are delivering quality care efficiently? What can we learn about crowd-out from state initiatives? Would individual and employer mandates make the plan more palatable? To address these and related questions, the Alliance for Health Reform and The Commonwealth Fund sponsored an April 27 briefing. 
10Briefing  Covering the Uninsured: Options for Reform, 3/2/2009
Forty-five million Americans were uninsured in 2007. They may have worked for an employer that didn’t offer coverage, or were eligible for coverage on the job but could not pay their share of the premiums. Different approaches are being considered for expanding coverage for the uninsured and underinsured in the context of health reform. Understanding the basics of these options, whether they build on the current system or propose to radically change the system, is important for moving the debate forward. Who are the uninsured and why aren’t they covered? How will each coverage option reduce the rolls of the uninsured? What difficulties might one encounter with particular options? To address these and related questions, the Alliance for Health Reform and the Kaiser Commission on Medicaid and the Uninsured sponsored a March 2 briefing. 
10Briefing  How Wide Has the Window Opened for Health Reform?, 12/11/2008
Now that we know the next occupants of the White House and the composition of the next Congress, many Americans expect to see an effort to enact significant reform of America’s health system. What are the prospects for expanding health coverage to the uninsured, given the recession? Will we see workable approaches to constraining health care costs and improving quality? Could the tax treatment of employer-sponsored health benefits change? Who will be the key players in health reform – in the Senate, the House and the Obama Administration? Can we avoid the hurdles encountered during previous efforts at significant health reform? This briefing, sponsored by the Alliance and the Robert Wood Johnson Foundation, addressed these and related questions.  
10Briefing  Lessons on Coverage from Abroad: How Do the Dutch, German and Swiss Health Care Systems Do It?, 11/14/2008
The pre-election debate put health care reform ideas front and center. Now policymakers have to deal with translating theory into action under challenging economic conditions. Why completely reinvent the wheel when there are existing universal coverage systems that may have components the U.S. can learn from? This briefing, cosponsored by The Commonwealth Fund, provided an in-depth look at the very different approaches of Switzerland, Germany and Holland to providing near-universal coverage to their citizens. How is coverage provided in each system? How successful is each in achieving maximum coverage? What are the roles of private insurance and government in each? What mechanisms are in place, if any, to help each system manage in difficult economic times? Panelists addressed these and related questions.  
10Briefing  Massachusetts Health Reform: A Giant Step Toward Universal Coverage? , 11/7/2008
The Massachusetts model, enacted in 2006, is being watched closely for lessons that might guide national efforts to reduce the number of uninsured and underinsured nationally. The ranks of the uninsured in Massachusetts have dropped dramatically, yet questions remain. Precisely, how is the pursuit of universal coverage progressing in Massachusetts? What impact, if any, are the reforms having on insurance costs? Are the newly insured able to actually get the care they need? Is subsidized coverage “crowding out” employer-sponsored coverage? How are employers responding to the reforms more generally? Can the state afford the subsidies as economic problems affect tax revenues? To address these and related questions, the Alliance for Health Reform and The Commonwealth Fund sponsored a November 7 luncheon briefing.

Hispanics will make up almost three out of every 10 people in the U.S. by 2050. What health concerns do Latinos have? What special prevention efforts are needed for this community? Will we have enough health professionals who are culturally competent to care for Latinos? How should national health reform proposals address the health needs of Latinos? Who should pay for the health care of undocumented immigrants without health coverage? This briefing, cosponsored by the Robert Wood Johnson Foundation, addressed these and related questions.  
10Briefing  Using Medicare as Part of Coverage Expansion?, 6/6/2008
A new proposal by The Commonwealth Fund suggests, as a major component of moving toward coverage for all, a Medicare-like program that uninsured adults below the age of 65 could purchase voluntarily. This briefing, co-sponsored by the Fund, featured speakers both favoring and skeptical about this approach.  
10Briefing  Putting the Brakes on Health Care Costs: Would the Candidates’ Plans Work? Are There Better Solutions? , 6/3/2008
Would the health plans of Sens. Obama and McCain curb health care spending? Doubtful, said three respected health economists speaking at this briefing cosponsored by the Robert Wood Johnson Foundation. Other solutions offer more cost-cutting potential, speakers said.  
10Briefing  Massachusetts Health Reform: Bragging Rights and Growing Pains, 5/19/2008
The Massachusetts health care reform plan has been a success in many ways, with more than 340,000 formerly uninsured residents gaining coverage. But the program is costing more than expected and the majority of the newly insured require hefty government subsidies that may not be sustainable. This briefing, cosponsored by the Kaiser Family Foundation, looked into the program's successes and challenges.

While the United States delivers some of the best medical care in the world, there are major inefficiencies in our system. What can we learn about coverage patterns and health care costs in other countries that might help inform our choices in the U.S.? Comparative effectiveness research holds out the tantalizing prospect of making it easier for patients and their doctors to choose the best treatment. Who should conduct such research -- government, the private sector or some combination? Who should decide which interventions to evaluate? Who will pay for it? This briefing, cosponsored by the Robert Wood Johnson Foundation, addressed these and related questions. 
10Briefing  Health Care in the 2008 Elections: Where Do the Candidates Stand on Promoting a High-Performance Health System?, 3/14/2008
This event featured advocates for the health plans of Sens. Clinton, Obama and McCain describing key features of their candidates' plans and how the plans differ. A leadoff presentation by Sara Collins of The Commonwealth Fund offered the Fund's guidance for health reform proposals, with estimates of future savings if efficiencies proposed by the Fund were to be combined with universal health coverage. Cosponsored by The Commonwealth Fund.  
10Briefing  Health Information Technology and Privacy: Is There a Path to Consensus? , 2/29/2008
Why do Americans trust information technology when they do their banking, make airline reservations, pay their bills, and file their tax returns, but not to access their medical information electronically? Some believe that technology poses a serious threat to Americans’ medical privacy rights; such people would add strict privacy protections to any health IT legislation. Others assert that the privacy risks are inconsequential, and that all health care providers should use electronic health records and participate in health information exchange networks. What’s the best way to balance the move to electronic health records and other health IT applications against the need to protect the privacy of individuals’ medical histories? This briefing, cosponsored with the Divided We Fail Initiative, addressed these and related questions.

With health care spending topping two trillion dollars in 2006, currently at 16 percent of gross domestic product and expected to rise to 20 percent by 2015, it is no surprise that most Americans are dissatisfied with the total cost of care. But who or what are the biggest culprits in a high cost system of care, and what can be done to contain costs? No matter who wins the White House and control of Congress in November, health reform legislation will likely be a front-burner issue for both House and Senate in 2009. The debates about reform, and the behind-the-scenes meetings, of 1993 and 1994 produced a wealth of knowledge on what should be done differently the next time Congress takes up this issue. What derailed health reform in 1994? What tactical and strategic decisions, in areas such as timing and content, affected the outcome? What part did outside groups play? How should the issue be approached differently the next time around? What are the appropriate roles for congressional and executive branch leadership in getting reform done? 10Briefing  STATE COVERAGE INITIATIVES: Will Moving Toward Universal Coverage Make the System Work Better for Everyone?, 10/26/2007
There is mounting evidence that the U.S. system is often financially inefficient and delivers poor quality. There is also research suggesting that having so many people without insurance can adversely affect the quality of care for everyone, even those with insurance. A new report by The Commonwealth Fund sets forth principles for providing universal coverage in ways that could promote a high performance health care system. Several states are already trying to reform their health care systems. How is implementation of universal coverage progressing in Massachusetts? How are negotiations moving forward in California and Pennsylvania to pass health care reform legislation? In addition to improving access and quality, what else should states do to improve the health and health care of their citizens? Employer-sponsored coverage rates have been steadily falling, from 66 percent of non-elderly Americans in 2000 to slightly below 60 percent in 2006. Experts cite rising premium costs and workforce changes as factors driving the erosion in such coverage. What are the pros and cons of employer-sponsored coverage? Who is losing coverage and why? Is it possible to maintain or improve health coverage for employees and also prevent jobs from moving overseas? This briefing, sponsored by the Alliance and the Robert Wood Johnson Foundation, explored these and related questions.  
10Briefing  HEALTH CARE and the '08 ELECTION: A Preview, 7/10/2007
As presidential candidates of both major parties woo the American public, particularly in early caucus and primary states, they are gravitating toward a common public concern: the U.S. health care system. Potential voters are well aware of rising premiums and higher out of pocket costs to families, the impact of high health care costs on America’s economic competitiveness and on federal and state budgets, and the ever-growing number of uninsured – now nearly 45 million. A Kaiser Family Foundation poll released in March shows that health care ranks as the second most important issue that the public would like discussed by presidential candidates for the 2008 election.

Young adults have among the highest uninsurance rates in the U.S. In 2008, three in 10 uninsured Americans – almost 14 million people – were between 19 and 29 years of age. As part of an ongoing effort to explore what is in the Patient Protection and Affordable Care Act and the Health Care and Education Reconciliation Act of 2010, this briefing dealt exclusively with how the reform law affects young adults. Unhealthy behavior is costing America billions in health care expenditures, and making us less healthy as a people. Many large employers, recognizing the impact on the health of their workers and the companies’ bottom lines, offer financial incentives to their employees to exercise regularly, improve their diets, lose weight and quit smoking, among other things. In the course of the health reform debate, the issue has arisen: which behaviors or health outcomes should be encouraged, and how large should the incentives be? What types of prevention models are working for business to keep workers healthier and restrain health costs? To answer these questions and others, the Alliance for Health Reform and AARP sponsored a December 7 briefing.

Medicaid and the Children’s Health Insurance Program (CHIP) play a crucial role in the U.S. health insurance system by providing coverage for more than one in four children. And yet, more than 8 million children remain uninsured today, 70 percent of whom are eligible for Medicaid or CHIP. What factors influence children’s coverage? How could the reform bills in Congress change children's coverage? To discuss these questions and others, the Alliance for Health Reform and the Robert Wood Johnson Foundation sponsored a November 13 briefing.

Health care delivery poses unique challenges in rural communities. Provider shortages are particularly acute in rural America, where levels of educational achievement are generally lower and there are fewer opportunities to receive medical training. Unemployment and uninsurance rates are higher in rural areas than elsewhere. What provisions in the various reform proposals affect rural health care? What particular challenges need to be overcome in order to improve care delivery in rural areas? What aspects of health reform will require special accommodation in rural areas? Are delivery systems in rural areas currently able to handle the influx of new patients coverage expansion would bring? How does HIT factor into rural reform? To answer these questions and more, the Alliance for Health Reform hosted a briefing with the Robert Wood Johnson Foundation on October 30.

To improve the health of communities and the general population, an array of health reformers, states and businesses alike are all looking to a range of prevention measures such as chronic disease management, alcohol and smoking cessation, and obesity programs. The hope is that these measures will also improve value and control costs. What types of prevention models are working for business and government? What impact on health have prevention programs had? Does the evidence prove that prevention can cut costs? What emphasis should be placed on community vs. clinical prevention? What improvements need to be made in public health infrastructure, workforce and training to improve prevention efforts? Do some workers benefit more than others from the current tax subsidies? Does altering the tax treatment of health insurance have the potential to expand or diminish coverage? It is widely accepted that the U.S. health care system, although touted by some as the “best system in the world,” has room for improvement. Care is often fragmented and lacks coordination, which compromises quality and efficiency. Incentives for providers have not caught up with the demographics of chronic care. Can health care be refocused from acute care services to the management of chronic conditions? Is payment reform the first step in restructuring health care delivery? What part can private integrated delivery systems play? Are different models needed in large group practices and smaller ones? What challenges face policymakers in the current climate of health care reform? There is an emerging consensus that health reform must encompass ways to improve quality and value in the system, and one of the prime targets for reform is the way care is delivered. Some health care organizations have combined thoughtful use of technology and better use of personnel to improve preventive care and treatment of chronic disease, obtain better outcomes, and offer additional job satisfaction to their health care workforce. This briefing, sponsored by the Alliance, Kaiser Permanente and the AFL-CIO, explored ways to enhance a team approach to care.

As a state-run program with broad federal guidelines, Medicaid covers health and long-term care services for scores of millions of low-income Americans. Conversations are well underway about the role of Medicaid as a vehicle for economic stimulus, and about its role in health coverage expansion proposals. Who is eligible for Medicaid? How is it administered? What benefits are covered? What is its role in long-term care? What role does the program play for Medicare beneficiaries? How is Medicaid financed? How much of the funding for the program comes from the federal government? How much do states pay for Medicaid and how much discretion do they have in determining who is covered, and for what services? To address these and related questions, the Alliance for Health Reform and the Kaiser Commission on Medicaid and the Uninsured sponsored a February 13 luncheon briefing.

An estimated 125 million individuals in the United States have a chronic illness, disability or functional limitation. Chronic diseases are the number one cause of death and disability in the United States and account for three quarters of the nation’s health spending. What models for improving care coordination exist and how can they be more widely implemented? Is there evidence that improved treatment of chronic care can reduce costs? Or increase costs? This briefing, cosponsored with Novartis and the National Institute for Health Care Management, addresses these and related questions.

Following up on last year’s national scorecard on the U.S. health system performance, The Commonwealth Fund has developed a state scorecard to assess state variations in key areas of access, quality, hospital use and cost, equity and health outcomes. Results of the state-level comparisons were reviewed and analyzed at a luncheon briefing sponsored by the Fund and the Alliance for Health Reform.
