Though some believe economic recovery is underway, Americans are still losing jobs by the hundreds of thousands each month. With the loss of jobs, so goes health insurance. How does this reality affect health reform and the notion of building on the current system? Has the complexion of who is uninsured changed? What has been the role of public programs in the recession? To address these and related questions, the Alliance for Health Reform and the Kaiser Family Foundation sponsored a September 18 briefing.

| 10 | Briefing | Covering the Uninsured: Options for Reform, 3/2/2009 Forty-five million Americans were uninsured in 2007. They may have worked for an employer that didn’t offer coverage, or were eligible for coverage on the job but could not pay their share of the premiums. Different approaches are being considered for expanding coverage for the uninsured and underinsured in the context of health reform. Understanding the basics of these options, whether they build on the current system or propose to radically change the system, is important for moving the debate forward. Who are the uninsured and why aren’t they covered? How will each coverage option reduce the rolls of the uninsured? What difficulties might one encounter with particular options? To address these and related questions, the Alliance for Health Reform and the Kaiser Commission on Medicaid and the Uninsured sponsored a March 2 briefing.

| 10 | Briefing | STATE HEALTH INITIATIVES: What's Next?, 5/30/2007 New initiatives are cropping up in one state after another aimed at helping the uninsured. The most comprehensive efforts have been in Massachusetts, Vermont and Maine. California Gov. Arnold Schwarzenegger has attracted national attention for his plan to cover virtually all in that state, and legislative leaders have their own plans. But many other states are also moving toward expanded coverage. This briefing presented a nationwide roundup, and thoughts on how state-level activity could affect prospects for national health reform. Resource materials are available for downloading, including an extensive list of expert sources.

| 10 | Briefing | Health Coverage Revisited: Exploring Options for Expansion, 3/19/2007 Providing health insurance coverage to the 46 million Americans without it has been receiving more attention in recent months. Several polls have shown rising public interest in the topic. Coverage is fast becoming an issue in the 2008 presidential campaign, for both Democrats and Republicans. President Bush offered an uninsured proposal in his January State of the Union address. Bills dealing with the uninsured have been introduced by members of Congress from both parties. A number of states have also laid out ambitious coverage plans.

Two new studies by John Holahan and Bowen Garrett of the Urban Institute offer in-depth analyses of the economic and demographic factors that underlie this troubling statistic.

| 9 | Toolkit | Toolkit: Crowd-out and SCHIP, 9/12/2007 Congress and the White House are gridlocked on SCHIP reauthorization beyond November. President Bush has vetoed the first SCHIP bill to cross his desk. CMS has issued tough new rules governing when states can open up their SCHIP programs to children above 200 percent of the federal poverty.
One reason for all of the above: crowd-out – the phenomenon of people dropping private health coverage when public coverage becomes more easily available.
This toolkit, supported by the Robert Wood Johnson Foundation, helps explain why crowd-out occurs and offers links to late-breaking developments affecting SCHIP enrollment. A list of experts and websites is also included.

They provide primary health care and other health services for medically underserved populations, including 1 in 8 Medicaid beneficiaries, 1 in 7 uninsured persons, 1 in 3 people in poverty, 1 in 10 minorities, and 1 in 9 rural Americans. Congress has now approved a stimulus package that includes almost $2 billion for health center infrastructure. It is timely, then, to look at how community health centers fit in today’s system, and how they might fit in a reformed system. Who is being served by CHCs today? What role do CHCs play in easing the widespread primary care shortage? What are the challenges facing CHCs as they strive to meet the growing need? How should we evaluate the benefit derived from spending the stimulus money? To address these and related questions, the Alliance for Health Reform, with support from the United Health Foundation and the National Association of Community Health Centers, sponsored a February 23 briefing.

Potential voters are well aware of rising premiums and higher out of pocket costs to families, the impact of high health care costs on America’s economic competitiveness and on federal and state budgets, and the ever-growing number of uninsured – now nearly 45 million. What to Watch for in the Coming Debates, 2/18/2005 President Bush’s FY 2006 budget could have long-lasting effects on several health programs. If enacted as is, the budget would trim $60 billion in the growth in Medicaid spending over the next 10 years. At the same time, the budget offers $11 billion in new money to enroll children in Medicaid and the State Children’s Health Insurance Program. It would provide $74 billion in tax incentives to help the uninsured buy coverage. Some congressional budget leaders have signaled their intention to find budget savings in another entitlement program – Medicare. After the hearings are over and the votes taken, how many of the president’s health initiatives will remain? What budget proposals originating with Congress might be adopted? How will discretionary programs – like the Centers for Disease Control and Prevention, and the National Institutes of Health – be affected? What are the long-term implications for health care providers and consumers, and for the deficit?

The ranks of the uninsured in Massachusetts have dropped dramatically, yet questions remain. Precisely, how is the pursuit of universal coverage progressing in Massachusetts? What impact, if any, are the reforms having on insurance costs? Are the newly insured able to actually get the care they need? Is subsidized coverage “crowding out” employer-sponsored coverage? How are employers responding to the reforms more generally? Can the state afford the subsidies as economic problems affect tax revenues? To address these and related questions, the Alliance for Health Reform and The Commonwealth Fund sponsored a November 7 luncheon briefing.

Fill the Coverage Gap?
, 9/15/2008 If you think that all poor Americans can get health coverage through Medicaid, think again. If they are not parents of dependent children, disabled or elderly, they do not qualify for Medicaid coverage, no matter how poor. As a result, more than half of all low-income uninsured persons are adults who do not fit in current eligibility categories as defined by federal law. Can Medicaid be redesigned to cover non-categorical poor adults? What lessons can be learned from states’ efforts to cover the uninsured through Medicaid expansions? Can private insurance fill the void? To address these and related questions, the Alliance for Health Reform and the AARP Public Policy Institute sponsored a September 15 briefing.
